POSITION SUMMARY

The Nurse Case Manager utilizes her clinical skills telephonically.

Fundamental components include but are not limited to:

The Nurse Case Manager is responsible for telephonically assessing, planning, implementing and coordinating all case management activities with members from our Federal Plans. The CM is responsible to evaluate the medical needs of the member in order to facilitate and promote the members overall wellness. The CM develops a proactive course of action to address issues presented to enhance the member's short and long term outcomes.

BACKGROUND/EXPERIENCE

RN with current unrestricted state license required

Willingness to obtain additional RN licenses once hired is required

3-5 years clinical practice experience is required

Case management experience preferred

EDUCATION

The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Registered Nurse is required
