The Population Health Specialist role is critical to the on-going expansion of our population health and emerging personalized health initiative. The Pop Health role will be responsible for working directly with our provider and community service partners to support transformation and achieving the triple aim through: achieving quality targets, managing complex members, identify emerging risk members and creating synergies with health plan team and the entire care ecosystem.

The Population Health Specialist will be part of the national shared services team working in the local health plan market responsible for managing relationships with key health and community service providers tied to our population health initiative. Key activities the PHS will be responsible for include:
Experience within an ACO and/or population health management within a provider organization or health plan- any experience with a new delivery model or healthcare transformation, patient-centered medical home, CMS/CMMI grant implementation, FQHC or rural clinic experience
Excellent written, verbal and presentation
Expert analytical and critical thinking skills strong desire to use analytics and data trends to drive care management, risk stratification and predictive modeling
Executive ready to take on increasing responsibility and leadership roles as key accountable care provider partnerships evolve and mature
Transformation specialist who understand the dynamics and challenges of helping our members navigate the healthcare system whether in a small community clinic setting or through a large integrated delivery system
Know the business of healthcare and understand that data-driven decisions tied to tightly aligned and coordinated teams will impact care
Ability to work fluently across various technological programs and platforms, analyzing data to support outcomes and drive effective care coordination
Expert project management skills- able to manage many relationships and initiatives simultaneously and achieve targets, deadlines and proactively drive day-to-day priorities
Experience with Medicaid or special needs populations
Deep understanding of integrated care management, public health, care transitions, population health management strategies, community-based care coordination models and clinical-decision-making tools
Creative thought leader who finds a way to get to yes with achievable outcomes, can overcome inertia and fear of change
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
