The Social Worker provides coaching, information and support in collaboration with the care manager to empower the member to make ongoing independent medical and/or healthy lifestyle choices. Adheres to care management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.

We are seeking a Social Worker to join the Aetna One Advocate team in New Albany, OH. The Social Worker will utilize advanced clinical judgment and critical thinking skills to facilitate appropriate physical and behavioral healthcare and social services for members through assessment and member-centered care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources, optimal member functioning, and cost-effective outcomes.
An LISW license is required
3-5 years of direct clinical practice experience post masters degree, e.g., hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility is required
Case management and discharge planning experience preferred
Managed care/utilization review experience preferred
Crisis intervention skills preferred
The minimum level of education desired for candidates in this position is a Master's degree.

Nursing/Licensed Social Worker (LCSW) is required
