This is a full time RN Assessor who will make visits to Aetna Members and complete needs assessments for personal care assistance services, adult and pediatric medical day care services, and MLTSS clinical eligibility.
This position requires nursing clinical judgment, travel and work at home computer data entry.
Assessment of Members:
Through the use of clinical assessment tools and information/data review, conducts comprehensive assessments of referred members needs/eligibility and determines clinical eligibility for requested services and/or meeting needs by evaluating members benefit plan and available internal and external programs/services.
Applies clinical judgment to the incorporation of strategies designed to reduce risk factors and address complex clinical indicators which impact care planning and resolution of member issues.
Assessor must ensure visit timeliness as per contractual requirements and be able to communicate needs in a professional manner to the case management teams.
Must have strong clinical documentation skills.
Required to have active non restricted RN License in NJ.
2+ years clinical practice experience is required
Position is field based and candidate Must be able to travel to members homes in assigned counties to conduct assessments.
Must be comfortable training in Princeton office for approximately up to 6 weeks.
Preferred background in care management, discharge planning, and/or clinical assessments.
Knowledge of electronic health documentation and MS office products is strongly preferred
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.
