POSITION SUMMARY

Bilingual Spanish field case manager needed to assist in coordinating care and assist in early appropriate and safe return to work.

TERRITORY IS Chicago, South Suburbs of Illinois, and North West, IN.

Fundamental Components but not limited to the following:

Travel is 50-75% to appointments to determine claimant status, treatment plan and timeframes to RTW. Coordination of rehab plan and assurance of compliance and progress Communication with all parties to keep everyone abreast of status and progress Evaluation of process, progress and initiating discussion to rectify any questions, concerns or issues Collaborating with different disciplines with focus on proper treatment, responses/progress to achieve safe RTW Documenting and reporting outcomes and events Researching materials to use for coaching and educating claimants on diagnosis, treatment and comorbidities.

BACKGROUND/EXPERIENCE:

Case management experience preferred but will train qualified candidates.

3+ years clinical practice experience. (Experienced in orthopedics, workers comp, neurosurgery, out patient surgery, occupational health, home health, UR, QA, medical/ surgical nursing, independent/self sufficient, organized, patient and likes to teach)

Bilingual Spanish required

Excellent analytical and problem solving skills

Effective communications, organizational, and interpersonal skills.

Ability to work independently (may require working from home).

Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.

Effective computer skills including navigating multiple systems and keyboarding

EDUCATION

The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse (RN) required in IL active and unrestricted

Must obtain IN RN license within 6-12 months.

CCM preferred.
