POSITION SUMMARY

This Registered Nurse (RN) Case Manager position is a great opportunity to join Aetna's National Account Care Management Solutions Team located in our Blue Bell, PA office. You'll have the opportunity to work in an exciting, fast pace atmosphere and administer a high touch integrated model for various designated customers. Please note that the schedule for this position is anticipated to be Monday through Friday standard business hours, with a once-per-week rotating evening shift until 7:00pm EST based upon business need.

Fundamental Job Components

The Nurse Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health care needs through communication and available resources to promote optimal, cost-effective outcomes. The CM process includes: assessing the member's health status and care coordination needs, inpatient review and discharge planning, developing and implementing the case management plan, ongoing evaluation of the case plan with Medical Director as indicated, exceptional disease education skills and case closure when closure criteria achieved. This position requires an RN with an unrestricted active license. Multi-state licensure may be required within first year of employment.

BACKGROUND/EXPERIENCE:Active and unrestricted Registered Nurse (RN) licensure required. 3-5+ years of licensed clinical practice experience as a Registered Nurse required. Current or prior experience within Case Management (managed care, home health, or hospital) preferred, but not required. Bachelor's Degree preferred, but not required. Certified Case Manager (CCM) certification a plus! Bilingual proficiency in Spanish a plus!

EDUCATION

The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
