This is an in-office position in Blue Bell, PA.

The RN Case Management utilizes their skills to collaborative assess, plan,facilitate care coordination, and advocate for options and services to meet an individuals and familys comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.

Fundamental Components:

The RN Case Manager will 1. Assess the member and family 2. Plan for their needs 3. Facilitates their care coordination 4. Advocate for options and services.


RN licensure is required

5+ years clinical experience required

1+ years of either oncology or hospice experience required

Case management experience preferred

Home health experience preferred

Bilingual preferred


The minimum level of education required for candidates in this position is a Bachelor's degree or equivalent experience.


Nursing/Registered Nurse (RN) is required

Nursing/Oncology Certified Nurse is desired

Nursing/Certified Case Manager is desired

Position has been approved-Replacement position.RN with 5 years experience in clinical setting preferably oncology with sound clinical assessment skills.
