The Field Case manager role offers the ability to work independently. This is a home based position, with 75% of your time spent in the field, local travel. This is a great opportunity to make a direct impact in member's lives!
The RN Field case manager is responsible for assessing and analyzing an injured employee to evaluate the medical and vocational needs required to facilitate the patients appropriate and timely return to work.
Fundamental Components Acts as a liaison with patient/family, employer, provider(s), insurance companies, and healthcare personnel. Interviews employees in their homes, work-sites or physician offices to provide ongoing case management services. Evaluates employee progress toward desired outcomes through assessment and evaluation. Communicates both in person and telephonically with employee, medical providers, attorneys, employers and insurance carriers. Prepares all required documentation of case work activities. Coordinates referrals to specialists and alternate treatment provides as appropriate. Regular local travel required.Reporting, daily (documenting while in the field can happen)
RN with current state nursing license required. (2-3 to years experience, clinical ICU or acute is preferred)Rehab experience would be a plus.Strong problem solving and analytical skills. Demonstrated computer skills. Strong communication, organizational and interpersonal skills. CCM preferred. Previous case management or home care experience preferred. (visiting nurses)Bilingual highly preferredStrong computer skills including Microsoft office and smart phone ability..
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
