POSITION SUMMARY

This opening is for the position of Clinical Care Manager for Mercy Care Plans Long Term Care program. Candidates must have a background in behavioral health and hold an independent license (LPC, LISAC, LCSW) through the Arizona Board of Behavioral Health Examiners. Candidates should also have a proven track record of working with diverse client populations in community mental health and/or other healthcare settings.

Fundamental Components:

This position utilizes critical thinking and judgment to collaborate and inform case management processes. More specifically, the position is responsible for facilitating appropriate healthcare outcomes for long term care members by providing care coordination, support and education through the use of care management resources. Further, this position will have a focus on providing case management services to members who have been identified as requiring more intensive behavioral health oversight. The Clinical Care Manager will also be responsible for traveling to members' homes, facilities and other places of residence in order to complete periodic assessments. Through this process the Clinical Care Manager assesses the members relative safety, functional needs, healthcare status and progress with receiving behavioral health services.

BACKGROUND/EXPERIENCE required:

  • LPC or LISAC or LCSW required

  • 2 years in case management experience required

    Preferred Skills:

  • Managed care experience preferred

  • Crisis intervention skills preferred

  • Critical areas to succeed - organization, critical thinking, collaboration and time management

    EDUCATION

    The highest level of education required for candidates in this position is a Master's degree.

    LICENSES AND CERTIFICATIONS

    Mental Health/Licensed Clinical Social Worker is desired

    Mental Health/Licensed Professional Counselor is desired

    FUNCTIONAL EXPERIENCES

    Functional - Medical Management/Medical Management - Case Management/1-3 Years

    TECHNOLOGY EXPERIENCES

    Technical - Computer Operations/System and Console Operations/1-3 Years/End User
