POSITION SUMMARY

This position is in a production environment where authorizations are created Via faxes or phones.

Fundamental Components:

Performs outbound calls to members to confirm follow up with post hospital discharge services via telephone.

Assists member with rescheduling of missed appointments and/or connecting member to an external clinic provider for both physical and mental health.

Follows up with providers to ensure members attend appointments or to engage providers in outreaching members.

Maintains accurate and complete documentation of required information that meets risk management, regulatory and accreditation requirements.

Protects the confidentiality of member information and adheres to company policies regarding confidentiality

BACKGROUND/EXPERIENCE desired:

2-4 years experience as a medical assistant, office assistant, 1-2 customer service experience preferred

EDUCATION

The highest level of education desired for candidates in this position is a High School diploma, G.E.D. or equivalent experience.
