Aetna Field Case Manager-Medical in Brooklyn, New York

Responsible for assessing and analyzing an injured employee to evaluate the medical and vocational needs required to facilitate the patients appropriate and timely return to work. Acts as a liaison with patient/family, employer, provider(s), insurance companies, and healthcare personnel. Interviews employee's in their homes, work-sites or physician offices to provide ongoing case management services. Evaluates employee progress toward desired outcomes through assessment and evaluation. Communicates both in person and telephonically with employee, medical providers, attorneys, employers and insurance carriers. Prepares all required documentation of case work activities. Coordinates referrals to specialists and alternate treatment provides as appropriate. Registered nurse with current state RN license. Strong problem solving and analytical skills. Demonstrated computer skills and use of smartphone. Regular local travel required, requires own car and valid drivers license. Strong communication, organizational and interpersonal skills. CCM preferred. Previous case management or home care experience preferred.

Current active and unrestricted NY RN License
CCM preferred, not required, but will be expected to obtain CCM certification when requirements met
Willingness to travel up to 80% of each work day
Familiar with smartphone, applications, Outlook, Windows 7
The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.

Nursing/Certified Case Manager (CCM) is desired
Nursing/Certified Disability Management Specialist (CDMS) is desired
Nursing/CRRN - Certified Rehabilitation Registered is desired
Nursing/Registered Nurse (RN) is required
Nursing/Case Management/1-3 Years
Nursing/Medical-Surgical Care/1-3 Years
Nursing/Home Health Care Ambulatory Nursing/1-3 Years
TechnicalExperience/TechnicalFocus/1-3 Years/End User
