POSITION SUMMARY

Manage caseload for the Medicaid Long Term Care population in Palm Beach County. Complete initial and routine comprehensive assessments for the purpose of identifying member care needs. Engage members routinely telephonically and in person through home/facility visits to coordinate services and community resources.

Bilingual in English/Spanish is required.

Fundamental Components:

Must have excellent communication, organization and time management skills. Must have Microsoft Office Suite and general computer knowledge

BACKGROUND/EXPERIENCE Required:

  • Case Management experience is required

  • Florida Department of Elder Affairs 701B Comprehensive Assessment Certification required (or willingness to obtain this certification)

  • Willingness to travel within designated geographical region as required

  • Bilingual English/Spanish required

    BACKGROUND/EXPERIENCE Preferred:

    Long Term Care Diversion Program experience

    EDUCATION

    The minimum level of education required for candidates in this position is a Bachelor's degree or equivalent experience.

    Telework Specifications:

    WAH: employee will primarily work from home office but will come into the local office for meetings. Field visits can consist of visit to member's home, place of residence, local hospitals, and physician offices.
