Aetna Case Manager in Albany, New York

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit design and/or acute health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license.

Fundamental components include, but are not limited to:

  • Understanding clinical impacts.

  • Providing solutions to constituent needs.

  • Providing education to promote health engagement.

  • Working closely with members to implement an individual plan of care.

    Qualification Requirements:

  • Minimum of 3-5 years of clinical practice experience required.

  • Active, unrestricted RN licensure for State of NY.

  • Flexibility to work until 7pm as needed (approximately once per week).

    Preferred Skills and Experience:

  • Case Management experience preferred.


    The minimum level of education preferred for candidates in this position is a Bachelor's degree or equivalent experience.

    Licenses and Certifications:

  • RN License required.

  • Certified Case Manager (CCM) is preferred.
